How Do I Become a Patient?

Any medical doctor, podiatrist, physician assistant or nurse practitioner can write a referral for physical therapy. The referral can be from the respective providers’ own referral pad, our individual referral pad or even on the referral pad of another physical therapy company.

Many insurances do not require that you have a referral to start physical therapy.  To find out if a referral is necessary for you to begin therapy, you can call us and we will verify this for you.

We would like to make it easy for you to find out how to become a patient with us.  You can always call us if you have any questions or concerns.  We would like to answer all of your questions before your first appointment with us.

We at Physical Therapy Professionals want to provide you, our patients, with the essential information you need prior to your visit.

What to Expect on my First Visit

Your physical therapist will have dedicated time for you and, in most circumstances, will first sit with you in a private room and discuss your symptoms with you.  Your therapist will review your medical history with you and begin to evaluate your condition.  Your therapist will make a list of impairments by using his or her clinical experience and education.  The therapist will then create realistic goals and give you his or her best estimation on how long it will take to meet the established goals. 

If time allows, the therapist will deliver treatment and may initiate a home exercise program.  If appropriate, you will schedule follow up appointments with the receptionist at the conclusion of the session. You may schedule your appointments in advance, if you wish, as this allows you to adjust your personal schedule.  The receptionist will provide a print out of your scheduled appointments for your records. 

Your therapist will start to type out your initial evaluation paperwork during or after your visit using electronic documentation.  In most cases, your therapist will send your paperwork to the referring provider’s office (if applicable) so that they may keep a record of your physical therapy visit in your file.

If the clothing that you are wearing is not conducive to the evaluation, we will provide you with a gown, shorts, or socks.  Our treatment rooms are private and we make every effort to maintain patient modesty.  We have also taken measures to insulate our treatment rooms so that no one else will be easily able to hear your discussion with your therapist.  Some patients prefer to be evaluated while in the open gym and we have this option available as well. Your therapist will be receptive to your preferences.

Patient Demographics:
This form will provide the office with necessary registration information such as your name, address, emergency contact, telephone number, etc.

Medical History:
This form will provide the office with necessary past and present medical history.

Acknowledgement of Receipt of Notice of Privacy Practices and Option to Give Access to Medical Records:
This form states that you will be offered a copy of our Notice of Privacy Practices paperwork at your initial visit and to list anyone you wish to have access to your medical records out of our office.

Please feel free to download and print this referral form and have your doctor fill it out for you.
